DO I HAVE A QUALIFYING CONDITION?

ALS, Anxiety, Cancer, Chronic Pain, Dysmenorrhea (Painful Cramps), Glaucoma, Inflammatory bowel disease, including Crohn’s disease, Intractable skeletal spasticity, Migraine, Multiple sclerosis, Muscular dystrophy, Opioid Use Disorder, Positive status for Human Immunodeficiency Virus (HIV) and Acquired Deficiency Syndrome (AIDS), Post-Traumatic Stress Disorder (PTSD), Seizure disorder, terminal illness with prognosis of less than 12 months to live, Tourette Syndrome are eligible for medicinal cannabis in NJ.

DO I HAVE PROOF OF THE CONDITION?

A prescription, patient portal screen shot, or any documentation from a medical professional or insurance company will suffice.

BOOK AN APPOINTMENT ONLINE

New patients will book online. You will also complete online forms and a provider will review your medical history.

COMPLETION!

Your online paperwork is completed. Our fee is paid. Your education session is complete. The provider will go over any drug interactions or concerns during your appointment. We recommend that moderate and high risk patients have a follow up visit, but this is not required.

You will now need to register on the NJ MMP or CRC website. The state cost is $10 if you choose a physical card.

GO TO THE DISPENSARY

The state reviewed and approved your application and you have a card - you made it! Check their website before going. Also we recommend calling for an orientation to the dispensary's process. Make sure you have all their resources too! Informed patients make Smart Cannabis patients.
